Based on what I am reading so far one uses the PCR to support the agglutination test. As the agglutination test sometimes tests positive for related strains of Brucella. As Bhai mentioned earlier if the symptoms, culture test and agglutination support the diagnosis along with the PCR as confirmation one should definitely cull the animal.

Polymerase chain reaction (PCR) was applied to confirm the diagnosis of brucellosis and to study its clearance in response to the standard treatment regimen with doxycycline and rifampin at hospitals in Callao and Lima, Peru. The PCR confirmed the diagnosis in 23 (91.7%) patients with brucellosis including 12 culture-confirmed cases. For patients treated at the hospital in Callao, PCR was positive for all samples collected during and at the conclusion of treatment and for 76.9% of follow-up samples collected on average 15.9 weeks after completion of treatment. For patients treated at the hospital in Lima, PCR tests were positive for 81.8% of samples collected during treatment, for 33.3% of samples collected at the conclusion of treatment, and for ≥ 50% of samples collected at first, second, and third post-treatment follow-up. Thus, Brucella DNA may persist in the serum weeks to months after completion of the standard treatment regimen.

இன்று பேஸ்புக் -இல் ஒரு நண்பர் நம்முடைய கால்நடை மருத்துவர்கள் சிலரின் நடத்தை பற்றி எழுதி இருந்தார்...மிகவும் அதிர்ச்சியாக இருந்தது.
சேலம் அருகில் ஒரு அரசு கால்நடை மருத்துவர் நேற்று காலை தற்கொலை செய்து கொண்டாராம்.  இப்போது புதிதாக நியமிக்கப்பட்ட கால்நடை மருத்துவர்களில் மூன்று பேர் கால்நடை மருத்துவ இயக்குனரகம் சென்று பிற்பகலில் இயக்குனரை சந்தித்து அந்த இறந்த கால்நடை மருத்துவர் பணி செய்த மருந்தகத்தில் தங்களை பணி அமர்த்துமாறு வேண்டி உள்ளனர். அந்த மருத்துவர் இறந்த விஷயம் தெரியாத இயக்குனர் அவர்களிடம் அங்கு இப்போது பணியிடம் காலி இல்லை என்று கூறி உள்ளார், அவர்கள் விடாமல் அந்த மருத்துவர் இன்று காலை இறந்து விட்டார் என கூறி உள்ளனர், அவர் உடனே சேலம் பகுதியில் பணி புரியும் மற்றொரு மருத்துவருக்கு தொலைபேசியில் அழைத்து விசாரித்துள்ளார், அவரும் அந்த கால்நடை மருத்துவர் தற்கொலை செய்து கொண்டதை உறுதி படுத்தி உள்ளார். இறந்த சக மருத்துவரின் இறுதி சடங்கு கூட நடை பெறவில்லை அதற்குள் அந்த இடத்திற்கு போட்டி போட்டுக்கொண்டு வந்து விட்டீர்களே என்று அவர்களை கடுமையாக திட்டி அனுப்பி உள்ளார்  இயக்குனர்....

PCR-Problems

There are two limitations of PCR assay:false-positive and false-negative results.
The ability of PCR to generate many copies of a sequence from minute quantities of template DNA necessitates that extreme care be taken to avoid the generation of false-positives.
Although false positives can result from sample -to -sample contamination ,a more serious source is the carry-over of DNA from a previous amplification reaction of the same target.To avoid such  false -positives ,specific precautions to control contamination must be taken .
To avoid  false-negatives ,it is important to control the presence of inhibitory compounds in blood samples,such as haeme compounds,anticoagulants and large amounts of total DNA .
In PCR for Brucella spp.,no standardization regarding sample preparation ,target sequence,PCR and detection methods has been established so far. 
My suggestion is that samples with a +ve blood culture but negative PCR ,should be considered as false PCR negative ;samples with a -ve blood culture but +ve PCR ,should be considered as false blood culture negative ,if there are clinical and serological findings to support the presence of brucellosis; samples with a -ve blood culture and negative PCR ,but with clinical and serological findings to support the presence of brucellosis should be considered as false blood culture negative and false PCR negative.


So, best results  can be obtained by combining culture 


and PCR ...

BRUCELLOSIS- AGAIN

பாய்  இப்ப PCR சோதனையில் நிபுணத்துவம் பெற்றிருப்பதால் அவருக்கு சில கேள்விகள்(என் சந்தேகத்தை போக்கிக்கொள்ள)

சில ஒட்டகங்கள் புருசெல்லா பரிசோதனையின் போது 1/1280 dilution positive வந்தது, பின்னர் சில நாட்களுக்குப்பின் திரும்பவும் சோதனை செய்தபோது அதில் ஒரு ஒட்டகம் மட்டும் 1/1280 dilution positive வந்தது மற்ற அனைத்து ஒட்டகங்களும் negative என்று பரிசோதனை கூடத்தில் கூறினார்கள். positive ஒட்டகத்தின்  ரத்தத்தை  PCR சோதனைக்கு உட்படுத்தியபோது negative என்று கூறினார்கள். கடந்த ஆறு மாதங்களில் சில ஒட்டகங்கள் positive வந்தன, நான் அவற்றை cull செய்யுமாறு கூறினேன், ஆனால் ஒட்டகங்களுக்கு பொறுப்பானவர்கள் அப்படி ஏதும் செய்ய வில்லை, இப்போது ஒட்டகத்தில் வேலை செய்யும் நான்கு பாகிஸ்தானிய பணியாட்களுக்கு அந்த நோய் வந்து விட்டது. மேலும் பால் கறப்பதற்காக வைத்துள்ள 26 ஒட்டகங்களில் நான்கு ஒட்டகங்கள் 1/1280 dilution positive (Ref. Method: Febrile Antigen Card method by Huddleson, I.GF and Bell Dis. 42 242), பாலை குடித்த முதலாளியின் குடும்ப உறுப்பினர்களில் ஒருவருக்கும் நோய் தாக்கி உள்ளது.
மேலும் பரிசோதனை கூடத்தின் முடிவுகள் மிகவும் erratic ஆக இருக்கிறது(IDAC LAB RIYADH) .ஒரே  ரத்த மாதிரிகளுக்கு ஒவ்வொரு நாளும் ஒவ்வொரு மாதிரியான ரிசல்ட் கொடுக்கிறார்கள். ஒரு நாள் 1/640 positive என்கிறார்கள் அதே ஒட்டகத்தின் ரத்தம் இரண்டு நாள் கழித்து அனுப்பினால் negative என்கிறார்கள். இதில் நான் என்ன மாதிரியான நடவடிக்கைகளை எடுக்கலாம் என்பது குறித்து சென்னை நட்சத்திரங்களின் ஆலோசனை/அறிவுரை வரவேற்கப்படுகிறது.

What a Doctor!

"The doctor was called one evening for an urgent surgery. He answered the call, changed to appropriate work clothes, reached hospital in minutes & went directly to the surgery block.

He found the boy's father waiting, in the hall for the doctor.

On seeing him, the dad yelled: "Why did you take all this time to come?
Don't you know that my son's life is in danger? Don't you have any sense of responsibility?”

The doctor smiled & said:
"I am sorry, I wasn't in the hospital & I came as fast as I could after receiving the call...... And now, I wish you'd calm down so that I can do my work"

"Calm down?! What if your son was in this room right now, would you calm down?

If your own son dies now what will you do??" said the father angrily

The doctor smiled again & replied:
"From dust we came & to dust we return, blessed be the name of God".

Doctors cannot prolong lives. Go & intercede for your son, we will do our best by God's grace"

"Giving advises when we're not concerned is so easy" Murmured the father.

The surgery took some hours after which the doctor went out happy, "Thank goodness!, your son is saved!" And without waiting for the father's reply he carried on his way running.  "If you have any question, ask the nurse!!"

"Why is he so arrogant? He couldn't wait some minutes so that I could ask about my son's state" Commented the father when seeing the nurse minutes after the doctor left.

The nurse answered, tears streaming down her face:
"His son died yesterday in a road accident, he was at the burial when we called him for your son's surgery. And now that he has saved your son's life, he is running to finish his son's burial.

"Never judge anyone... because you never know how their life is & what they're going through!!!

பயணத்தின் ஊடே பாய்...

மேலே உள்ள படங்கள்,தாயகம் வந்திருந்த போது, சென்னை-பெங்களூரு- சென்னை மார்க்கத்தில் ,முன்பதிவு செய்த கோச்சில் செல்லும் பொது எடுத்தவை.கடுமையான  கூட்டம்.ஏன்டா ஏறினோம் ? என்றாகி விட்டது.டிக்கெட் பரிசோதகர் ,செல்வதற்கு கூட இடமில்லை.அவரோ,முன்பதிவு செய்த பயணிகளின் டிக்கெட்டை மட்டும் பரிசோதித்து விட்டு ,ரொம்ப கஷ்டப்பட்டு முன்னேறி சென்று கொண்டிருந்தார்.அவரது,பணியை சரிவர செய்யவில்லை.என் மகள்,என்னை ஃ போட்டோ எடுத்தது கூட தெரியவில்லை. வழக்கமாக,இந்த மார்க்கத்தில் ஏ.ஸீ.கோச்சில் தான் செல்லுவோம்.இந்த முறை,வேறு வழியில்லாமல் சென்றோம்.ஆகவே,நண்ப-நண்பிகள்,

குடும்பத்துடன் செல்வதாக இருந்தால்,இந்த மார்க்கத்தில் ஏ.ஸீ.கோச்சில் மட்டும் செல்லவும்.

கீழே உள்ள படங்கள்,தாயகம் வந்திருந்த போது, சென்னை-நெல்லை - சென்னை மார்க்கத்தில் ,முன்பதிவு செய்த கோச்சில் செல்லும் பொது எடுத்தவை.சொகுசாக ,இடைஞ்சல் இல்லாமல் இருந்தது.டிக்கெட் பரிசோதகர் ,செவ்வனே  பணியை செய்தார்.எந்த அளவுக்கென்றால்,கும்பகோணத்தில் ,நடு இரவில்,முன்பதிவு செய்யாமல் ,எங்கள் கோச்சில் ஏறிய ஒரு சிறிய குடும்பத்தினரை,மயிலாடுதுறையில் இறக்கி விட்டு விட்டார்.
